About your new role
The Mental Health Clinician (MHC) provides high quality consumer care as part of a multidisciplinary team. The role delivers person centred services to consumers with a range of mental health needs, supporting wellbeing, safety and positive health outcomes across community and/or clinical settings.
Practising autonomously within their professional scope, the MHC undertakes assessment, care planning, implementation, evaluation, and coordination for consumer care. The role integrates therapeutic engagement and health education, while promoting trauma informed, and culturally safe practice. The MHC contributes to service quality, safety, and continuous improvement initiatives, and may provide clinical guidance and oversight where relevant to the program.
MHC provides clinical assessmentand suicide risk management within an outreach service model. The role is primarily focused on initial assessments conducted jointly with a nonclinical key worker in community setting, with the clinician responsible for assessing mental health needs, delivering a collaborative safety planning, identifying and managing suicide risk, and providing clinical guidance.
MHC may also provide additional follow up visits where clinically indicated, while maintaining a consultative and support role to nonclinical staff.
MHC assists non-clinical staff with ongoing assessing and managing suicide risk and any emotional distress.
MHC arranges outbound referrals and warm handover of consumers should theyrequire escalation of support into clinical services during their engagement with Aftercareservice.
